Expect a phone call from the secretary during the late afternoon the day prior to your scheduled visit. Please let the secretary know the preferred contact method (i.e. phone or e-mail) to which you prefer to be contacted. Indicate your prefrences at the end of the New Patient Questionaire if you are a new patient.
All appointments are made only with the secretary by calling 212-688-7100, prompt 4. When arriving at the office for any appointment, you should check in immediately and fill out eight items on the top of the progress note form. This is required by insurance for each and every visit. Please note: Item number 8 asks for a list of medications you are currently taking along with dosing information. Please bring a list of your medications with you. After the completed form is returned to the secretary, you will have your blood collected by either the medical assistant or the nurse. Dr. Wolf will also see you if you are scheduled to see him.
If any special instructions are required for your initial visit, the secretary will instruct you to do so at the time the appointment is scheduled. At the time of confirming your appointment, the day before your scheduled visit, you will be reminded of any specifics. Remember to bring medical records with you or have them sent proir to your arrival. To obtain a New Patient Form (NPF), click here. Please fill out the form and return it via e-mail secretary@djwolfmd.com, fax (212) 308-5242, or U.S. Mail. After the form is reviewed, you will receive a call back from the secretary to schedule your appointment. A New Patient questionnaire will be sent for you to complete before Dr. Wolf sees you. Please complete the New Patient questionnaire before coming to the office. Please specify to the secretary how you wish to obtain the questionnaire (via fax, U.S. Mail or arriving 20 minutes early to complete its contents). Upon arriving at the office for your new patient appointment, you should check in promptly with the secretary who will take your seven-page completed questionnaire (or give you one to complete if you have not already done so). You will also be asked to provide Alla, the medical biller, with your insurance card(s). You must also complete the items at the top of the progress note which is required by insurance for each and every visit. After the completed progress note form is returned to the secretary, you will have your blood collected by the medical assistant, after which you will meet with Dr. Wolf.
Blood Work - The medical assistant will collect your blood after you check in with the secretary after filling out the eight items on the top of a progress note. This is required by insurance for each and every visit. You will then see the nurse for a subcutaneous injection if needed. If not, you then check out with the secretary to schedule future visits or tests and, if necessary, also check out with the biller to pay and/or to discuss any billing questions or problems.
INR Patients - If you have had an INR performed by fingerstick, expect a call later the same day or in the evening for your Coumadin dosing instructions and to schedule your next INR. If, for any reason, you do not receive a call the same day, please contact the secretary at 8:30 a.m. the following day.

Fasting Office Visit or Fasting Blood Work - If you are asked to fast, you must refrain from eating or drinking for at least 12 hours prior to your scheduled appointment. You may drink water, black coffee, or tea without milk or sugar. Your being well-hydrated makes blood collecting easier. You should also take your usual medications. After checking in with the secretary, and filling out eight items on the top of the progress note, it is returned to the secretary. You will then have your blood collected by the medical assistant, who will escort you to an exam room. Gowns are provided for women and are given to men only upon request. Dr. Wolf will then examine you, after which you must check out with both the secretary for future scheduling and also with the medical biller to pay and/or discuss any billing-related questions or problems. If a fasting office visit is scheduled, you may eat after your blood is collected. You may choose to bring a snack with you. If you are scheduled for fasting blood work only, you must check out with the secretary and, if necessary, the biller after your blood has been collected.
Complete Physical Examination - Please arrive for your visit after refraining from eating or drinking at least 12 hours prior to your scheduled appointment time. You may drink water and drink black coffee, tea with no milk or sugar. Your being well-hydrated makes blood collecting easy. Please take your usual medications. After checking in with the secretary, and filling out the eight items on the top of the progress note, it is returned to the secretary. You will then have your blood collected by the medical assistant, who will escort you to an exam room. Gowns are provided for women and are given to men only upon request. Dr. Wolf will then examine you after which you will check out with both the secretary to review future scheduling and, if necessary, with the medical biller to pay and/or discuss any billing-related questions or problems.
Preoperative Clearance - The purpose of this visit is to obtain clearance for a surgical procedure. If you have a primary care physician, he or she should perform your preoperative clearance. In the event that Dr. Wolf is appointed by your surgeon to clear you for a surgical procedure, there is no fasting involved unless specifically requested by the surgical office staff. All paper work and tests that your surgeon requests must be sent to Dr. Wolf via fax, phone or e-mail. After checking in with the secretary, your blood will be collected by the medical assistant who will then escort you to an exam room for an EKG and physical examination. You must undress down to your underwear. Gowns are provided for women and are given to men only upon request. Dr. Wolf will then examine you. You must then check out with the secretary to review future scheduling and, if needed, with the medical biller to pay and/or discuss any billing-related questions or problems.
Treatments - Please first check in with the secretary. She will give you a progress note to complete. She will then notify Robin, the nurse, that you have arrived and are ready to be treated. Please wait in the waiting area until you are called by the nurse. Most treatments don't require special preparations. You should be notified if there are any preparations or medications to take before arriving at the office by Dr. Wolf or the nurse. Before, during or after your treatment, you will be seen by Dr. Wolf. When treatment is completed, please check out with the secretary and biller. If you receive sedating medications while in the office, you must make prior arrangements for someone to accompany you upon leaving.
