New Patients


New Patient Information

If you are a new patient, please click here to submit a New Patient Form (NPF) directly online. A new patient form can also be faxed to 212 308-5242 or U.S. mailed. You will then be phoned or e-mailed the same or next business day to schedule your new patient consultation appointment.

If you have commercial insurance, the initial consultation fee is $680.00. Payment is expected at the time of your visit. This is not required from Medicare patients. Medicare patients without secondary insurance are responsible for 20% of the Medicare-dictated charge to be paid at the time of your visit.

Insurance

Dr. Wolf accepts all commercial insurance plans with out-of-network benefits. He also participates in Medicare. If you have out-of-network benefits or Medicare plus a secondary insurance, the Billing Department routinely will submit your claims for you as a courtesy.


Scheduling

Do not schedule appointments too closely after visiting Dr. Wolf. Emergencies may back up his schedule. Please arrive about 15 minutes early for your appointment to complete the eight items on our progress note which must be done at each and every visit. This is required by Medicare and/or your insurance carrier. Please be aware that arriving early does not mean you will be seen any sooner by Dr. Wolf. If you cannot read or speak English, please have someone accompany you to translate and help fill out written forms.

Dr. Wolf is available to see patients Monday, Wednesday and Fridays from 8:30 a.m.-4:30 p.m. and Tuesday and Thursdays from 11:30 a.m.-4:30 p.m.


New Patient Appointments

If any special instructions are required for your initial visit, the secretary will instruct you to do so at the time the appointment is scheduled. At the time of confirming your appointment, the day before your scheduled visit, you will be reminded of any specifics. Remember to bring medical records with you.

After the form is reviewed, you will receive a call back from the secretary to schedule your appointment. A New Patient Questionnaire must be completed before Dr. Wolf sees you. Please complete the questionnaire before coming to the office. Upon arriving at the office for your new patient appointment, you should check in promptly with the secretary who will take your nine-page completed questionnaire (or give you one to complete if you have not already done so). You will also be asked to provide Alla, the medical biller, with your insurance card(s). You must also complete the items at the top of the progress note which is required by insurance for each and every visit. Please note: Item number 8 asks for a list of medications you are currently taking along with dosing information. Please bring a list of your medications with you for your visit. After the completed progress note form is returned to the secretary, you will have your blood collected by the medical assistant, after which you will meet with Dr. Wolf.

Confirmation of Appointments

Expect a phone call from the secretary during the late afternoon the day prior to your scheduled visit. Please let the secretary know the preferred contact method (i.e. phone or email) to which you prefer to be contacted. Also this method should be indicated on the New Patient Questionnaire.

Cancellation Policy

The office has a cancellation policy. Patients must cancel more than 24 hours in advance of a scheduled appointment. For new patients the cancellation fee is $250. For an established patient, the penalty fee is the cost of the missed scheduled appointment.
